Thank you for showing interest in becoming a Vendor with Vantage Point Title. Please provide the requested information below attach any and all documents or insurances related to your services.  Your information is added to our system upon receipt.

If you wish to be added as an Abstractor: Please provide a completed and signed W-9 and a current (unexpired) E& O Insurance

We will need the following information:

1. Business name or full name you would like your orders addressed to and your payment will be issued in this name.
2. Phone number\Fax number;
3. Email all correspondence will go to.
4. Billing address
5. Counties covered
6. Standard fees

If you wish to be added as a Notary: Please provide a completed, signed W-9, current (unexpired) E&O Insurance, and Background Screening. States: Indiana and Maryland: copy of License is also required
